Flu Prevention
On average in the United States, children miss approximately 38 million school days every year due to influenza. Influenza, also known as the flu, can be a serious illness and each flu season is unpredictable. The single best way to protect against flu and its potentially severe complications is to get a seasonal flu vaccine each year. The CDC recommends that everyone 6 months of age and older get a seasonal flu vaccine each year, preferably before the end of October. Getting vaccinated isn’t the only way to protect yourself and your loved ones. Watch this video to find all of the ways you can prevent the spread of the flu or other viral illnesses.

Immunizations
Wyoming has a state-wide immunization registry system that I have access to. In order to be able to check your childs immunization status via the system, I need to have your written permission to do this. Here is a link to the form if you wish for me to do this: https://files.gabbart.com/1407/immunization_agreement.pdf
The medication administration authorization form must be completed fully in order for the school to administer any medication. A new form must be completed at the beginning of every school year, for each medication and each time there is a change in dosage or time of administration of medication. An adult must bring in any form of medication to the school nurse or the school office, along with the form below. All prescription medications must be in the container given to you by the pharmacist or prescriber, and non-prescription medication must be in the original container with the label intact. Please download the form to the left if you need.
